TRANSITIONING TO A NEW OFFICE
 It will take some time to sort through everything in the office, please be patient.
All of the records are being sent to ASSOCIATED EYE CARE.  They will maintain the records for me and provide access to the information you might need to you and your next eye doctor.  If you have an appointment at Associated Eye Care in Sanford, they will scan what they need into their records system.  Be sure to let them know you were my patient when you make the appointment.  If you go to one of their other locations (Waterboro or Kennebunk), please let them know ahead of time so they can forward the records to the satellite office.  If you go to any other optometrist office, just call Associated and they will forward the information before your appointment.  If you need your records before they get sent to Associated, call my regular number, and we will forward the info for you.  If you don't get us directly, please leave a message on the answering machine and we will get it when we are there.  Please provide patient's name, birthdate and the name of the office you are going to.
